I'm about to seal the deal on a motorcycle, and since it's a 2001 model I need an odometer disclosure statement for when I register it (as says the NCDMV).

Is there a specific form given out from the DMV, or can it just be a piece of paper stating the mileage and signed by the seller and buyer?

should have a box on the back of the title that stats "odometer" reading and you just fill in the current mileage. They really don't care about it. Hell, I've left it blank before and they didn't care.

i traded a guy cars.. and at the dmv we just filled out the odo part.
