well i'm no APA probut seeing as how that questionnaire is a webpage, i would reference it as i would any other web page

In this case, (Engs, 2002) is indicated.and just for kicks, the biblio would look something like:Engs, Ruth C. (2002). The Student Alcohol Questionaire.
