if i have a custom device (not a pc) that has a usb port on it, could i get a usb -> bluetooth adapter and expect it to work out of the box, or do these adapters require drivers to work?

What do you mean *custom* device?

A small microcontroller with USB port?

In either case you will need drivers to use bluetooth.
