He doesn't use computers oftern because of his war injuries, not because he doesn't understand their importance in modern life. Indeed, Jacob Weisberg said that McCain was the most "cyber savvy" of the 2000 candidates (a list that included inventor of the internet, Al Gore). McCain gets by with help from his wife and aids. I'm sure in a world of limitless resources and time he could have found a computer setup that might suit his injuries (or potentially have something custom made) and learned how to use that unique set up.
