I just got a call from the lady's husband - she was just released from the hospital this morning - she was having a mini stroke of sorts due to constriction and a small blood clot in her head - because she got medical attention as quickly as she did, she is expected to fully recover in a few days. I feel so much better now that she is ok. The husband also asked for my address so he could send me an early christmas present! Thank God my parents raised me the way they did, thank God I acted the way I did, and thank God the woman survived.Merry Christmas TWW.

Because I don't want this post misconstrued as an attempt to get additional 'at a boys', i am making this small.i've actually been the first person on scene at a number of emergencies, one involving a man that was in the middle of a seizure, another was similar to this one, involving a woman who pulled into a sheetz gas station where my brother and i were eating breakfast - her hands were stiff as boards, and she had a crazed look of desperation, going through a panic attack. i held her hand for a good 15 minutes waiting on ems (it was rush hour @ aviation parkway to be fair to the ems guys). in both cases the people ended up being fine.]]

i saved a life tuesdaysort of, well ok, prolonged a life another day.i was with my own patient when i heard the "warning" alarm go off on the central monitor. It was going off for a few minutes and no one was responding to it... so i go down to see what the alarm was about. someone's HR was 40 (not always a big deal) till i saw what her ICP(intracranial pressure was) it was 60 holy shit!! thats pretty high. the house officer was there and since the patients brain drain (EVD) was clamped i asked her if i could drain some spinal fluid off. She said no, only for a neuro change i replied even if the patient is clearly cushing(google cushings triad)? Neurosurgery said only for a neuro change... whats her pupil look like. I ran to get my penlight popped open her eyes... one was fixed and dilated and the other was a fucking triangle. HO then says i can drain her down. i drained off some CSF dropped her ICP back into the high teens and within a few minutes her pupils were equal and reactive again --- yay saved a life.... then i went and told the new grad nurse that ignored the alarm that her patient almost herniated.....

^^ what's a neuro change? and did you ever figure out the cause of the ICP? do a CSF culture or anything? shouldnt this patient have a shunt?

neuro change. change in patients neuro exambaseline for her was globaly aphasic... would withdraw flex to central paininitial cause of high ICP was large thalamic ICH with 15 mm midline shiftover the next few days she had lessening midline shift, but then had more edema and went back up to 13mm midline shift, this was when i had her as a pt over a week ago when we began to induce hypothermia for icp control, now i have no idea what the treatment plan is, i think the family was going to withdraw care on her friday, but im not sure. she had a poor prognosis from the begining. neurosurgery wanted the ventric out, so they wanted it clamped for 24 hours, she proved tues night that clamping her drain would lead to herniation with the quickness. she would have needed a shunt but nsu is loathe to make people shunt dependent.
